Please complete this form to nominate an individual who has demonstrated hard work and dedication to the American Black Chiropractic Association (ABCA) and the Chiropractic profession within the current year.

A total of two (2) awards will be presented this year in the month of June: Doctor of the Year & Student of the Year

Nominees should meet the following guidelines:

A. Be a current, dues-paying member of the ABCA/SABCA.
B. Be directly and effectively involved in ABCA affairs and activities by dedicating time and insight to help the association uphold it’s mission statement and purpose.
C. Is a credible role model professionally and personally to his/her community, to other health professionals, and to current students of Chiropractic.
D. Effectively represents the Chiropractic profession.
E. Must not have been awarded ABCA Doctor of the Year within the last 5 years. Students who have been awarded Student of the Year are not eligible to receive the award a second time.

Nominators for awards should be current dues-paying members of the ABCA/SABCA.
